Introduction: Older adults in self-managed nursing homes are vulnerable to diabetes mellitus because structured health promotion and prevention programs are not yet available, while psychosocial stress can worsen their health conditions. This community service program aimed to improve healthy lifestyle behaviors and psychological well-being among older adults through the WREDA-PERMA program at Yayasan Panti Jompo Wijaya Kusuma, Bekasi City. Method: The program used a descriptive quantitative approach with a pretest-posttest comparison, conducted from June to August 2026 among nine older adults selected by purposive sampling, assisted by seven caregivers and two nursing home managers. The activities comprised health screening, relaxation techniques (Positive Emotion), hydroponic gardening (Engagement), “Isi Piringku” nutrition education (Relationships), story sharing (Meaning), and recognition of achievements across the program (Accomplishment). The data were analyzed descriptively by comparing results before and after the program. Results: The mean Mini-Mental State Examination (MMSE) score increased from 20.56 to 22.33, and the mean Mini Nutritional Assessment (MNA) score rose from 20.72 to 22.44, whereas blood glucose levels varied among participants. Conclusion: The PERMA program is feasible to continue, provided that the role of the nursing home management is strengthened.
